SUMMARY:
The community health nurse provides comprehensive, culturally safe, and client-centered nursing care to individuals, families, and the community of Timiskaming First Nation. The CHN supports holistic health and wellness by integrating physical, emotional, mental, and spiritual care, aligned with community values and traditions.
KNOWLEDGE AND REQUIREMENTS:
• Strong clinical assessment and critical thinking skills.
• Knowledge of community health, public health, and primary care practices.
• Understanding of trauma-informed care and harm reduction principles.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Good written and spoken communication skills in English, French is an asset
EDUCATION AND EXPERIENCE REQUIREMENTS:
• Bachelor of Science in Nursing degree with a current registration in good standing with the Ordre des infirmières et infirmiers du Québec (OIIQ).
• Current CPR and First Aid certification (BLS for Healthcare Providers).
• Minimum 1–3 years of nursing experience (community or primary care preferred).
• Experience working with Indigenous communities is considered an asset.
• Experience working with an EMR system is an asset
MAJOR RESPONSIBILITIES:
Clinical Care
• Provide direct nursing care including assessment, planning, implementation, and evaluation of care.
• Administer medications, treatments, and immunizations in accordance with professional standards.
• Perform health assessments, screenings, and follow-ups.
• Respond to urgent and emergent health concerns within scope of practice.
• Maintain accurate and timely documentation of all client interactions.
Primary Care & Chronic Disease Management
• Support clients with chronic conditions (e.g., diabetes, hypertension, mental health conditions).
• Monitor and manage care plans in collaboration with physicians and other healthcare providers and social service workers.
• Provide education on disease prevention and self-management.
Community Health & Health Promotion
• Deliver community-based programs (e.g., immunization clinics, prenatal care, wellness clinics).
• Participate in health education initiatives addressing topics such as nutrition, mental health, substance use, and injury prevention.
• Conduct home visits as required.
