The Temagami Family Health Team (TFHT) is recruiting a part-time Nurse Practitioner for 2 days per week.
The Temagami Family Health Teams is a collaborative inter-disciplinary team of physicians and primary health care providers with a vision to enhance primary health care, improve patient access and integrate a collaborative care model for patients. The Temagami Family Health Team is committed to providing safe and accessible patient-centered care.
We are currently seeking a motivated and organized Nurse Practitioner to join our team 2 days per week at our Temagami clinic location.
The Nurse Practitioner role is an integral part of the interdisciplinary health care team, delivering nursing care to a variety of patient populations including high-risk groups with chronic disease and/or complex multi-system diseases.
Key Responsibilities include, but are not limited to:
- Work collaboratively with all members of the TFHT in providing direct primary care, and participating in the development, implementation and promotion of health and prevention of disease.
- Lead primary care episodic appointments, increasing access to enhanced assessment, diagnosis, and treatment within the community and/or target population.
- Perform individual assessment, diagnosis, treatment, health education/counseling, and other clinical activities according to the College of Nurses in Extended Class (NPs)
- Consult as appropriate, with members of an interdisciplinary team, referring primary care provider, the local hospital, community care partners, specialists, and the surrounding community to assess, plan, implement, evaluate, and coordinate a patient’s plan of care and treatment.
- Provide health education and health promotion that are responsive to client needs.
- Collaborate with clients and families as well as the multidisciplinary team to develop client care plans.
- Communicate with the physician regarding patient needs, the nursing assessments, and recommendations.
- Facilitate client referrals to appropriate internal and external health and social services, including diagnostic services, support programs, and community-based health agencies.
- Play a leadership role in supporting the planning, implementation, and evaluation of community based initiatives.
- Actively participates in staff and team meetings as appropriate to facilitate program development and implementation
- Participate in quality improvement initiatives and organization-wide committees.
Qualifications:
- Current Registration (in good standing) with the College of Nurses of Ontario (CNO) in the Nurse Practitioner - Extended Class - Primary Health Care.
- Graduate of a recognized Primary Health Care Nurse Practitioner program.
- 2 years of clinical experience as a Nurse Practitioner preferably within a multidisciplinary primary care setting, preferred.
- Current Basic Life Support (BLS) certification.
- Completion of police record/vulnerable sector check upon hire.
Job Specifications:
- Part-time – 2 days per week
