POSITION SUMMARY:
The Home & Community Care (H&CC) Visiting Nurse provides nursing care in clients’ homes and the clinic, working with the H&CC Case Manager Nurse and support staff to help clients maintain independence. This position reports to the Medical Services Manager.

PREFERRED QUALIFICATIONS:
• Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
• Membership with RNAO or WeRPN, including LAP insurance
• Minimum two years of clinical nursing experience
• Current CPR, AED, and First Aid certification, or willingness to obtain
• Strong interpersonal and communication skills in a clinical setting
• Strong computer and organizational skills, including use of office equipment
• Ability to speak Ojibway, Cree, or Oji-Cree is an asset
• Valid driver’s licence required
• Satisfactory criminal background check required

DUTIES AND RESPONSIBILITIES:
• Provide nursing care in home and clinic settings, including IV therapy, wound care, medication administration, and phlebotomy
• Assess clients and help develop and implement individualized care plans with the HCC Case Manager Nurse
• Maintain accurate documentation and update client records in Mustimuhw cEMR in accordance with CNO standards
• Promote client independence through self-care, chronic disease management, and prevention education
• Support clients and caregivers in providing safe care at home
• Coordinate referrals and collaborate with healthcare providers to support integrated care
• Advocate for clients and support access to health and community services
• Participate in team meetings, program planning, and quality improvement
• Support program operations, including scheduling and coverage for the Case Manager Nurse as needed

WORKING CONDITIONS:
• Combination of clinic-based and home visits
• Travel within the community required
• Exposure to a variety of home environments and health conditions
