Social Worker - Out-Patient Mental Health Services

POSITION SUMMARY:To provide Counseling services to clients referred to the Mental Health Program. 

POSITION RESPONSIBILITIES 

  • To know and function within standards of professional competence.
  • To participate as a member of a multi-disciplinary health care team of Mental Health Program.
  • To complete psycho social assessments and plan for effective intervention strategies for clients diagnosed with a serious mental illness.
  • To evaluate the psychosocial situation through interviews with the client, family members and/or friends.
  • To provide individual psychotherapy for referred clients.
  • To develop or facilitate group programs.
  • To inform the multi-disciplinary team and relevant community agencies of any changes that may affect them.
  • To collect data for client tracking and performance indicator purposes.
  • To consult regularly with members of the multi-disciplinary team regarding client status and service needs.
  • To maintain effective public relations by making mental health resources known to the community and by utilizing resources of the community for the benefit of the client.
  • To recognize any physical or emotional problem outside the scope of Social Work knowledge and refer to another professional staff member.
  • Other duties as assigned.

POSITION REQUIREMENTS: 

  • Master’s in Social Work.
  • Must be certified with the Ontario College of Certified Social Workers and Social Service Workers.
  • Excellent clinical assessment skills.
  • Working knowledge of mental health legislation and related policies.
  • Working knowledge of the signs and symptoms of mental illness, risk indicators, and the potential impact of mental illness on family members.
  • Knowledge in CBT and DBT required
  • Working knowledge of the relevant services within the mental health program and the community.
  • Must possess the verbal ability to communicate with patients, physicians, program staff and other stakeholders in order to give and receive information or instructions and to write clear and accurate reports.
  • Must have the ability to function as a member of a multi-disciplinary health care team.
  • Employment history which demonstrates regular attendance at work.

Bilingualism (French and English) would be considered an asset.

  • Must be vaccinated against COVID-19.
