Living Space staff work collaboratively to provide person-centered services in a flexible, non-judgmental, compassionate, and responsible manner. The position supports individuals who experience persistent and chronic homelessness with a focus on supporting them to achieve housing stability. The position works collaboratively with community partners and across the different programs of Living Space to ensure that individuals experiencing homelessness/at risk of homelessness (EH/ARH) needs are being met and that each individual is supported to secure and maintain housing of their choice.
- Support individuals to secure housing of their choice
- Provide intensive support to individuals being rapidly housed and ensure coordination of intensive support including regular, continuous contact with the individual for three months, and periodical follow up for one year.
- Work collaboratively with partners of the Housing Now program to support landlords as necessary in conjunction with the individual served.
- Assist individuals to complete a VI-SPDAT and work toward housing focussed goals based on their strengths, abilities, and preferences.
- Create or update individual’s crisis plan.
- Advocate, refer and connect individuals with appropriate services, supports, and resources (formal and informal) based on their needs and choice.
- Champion each individual’s capacity for independent community living.
- Work collaboratively with partners of the Housing Now program to assist individuals to successfully remain housed, providing prevention and diversion strategies to avoid homelessness.
- Work collaboratively with partners of the Housing Now program to provide crisis prevention and intervention; reducing the utilization of EMS, ER visits, days in hospital, days incarcerated.
- Experience working in a cross-cultural environment.
- Experience conducting outreach to and advocating for marginalized individuals.
- Demonstrated knowledge of issues related to poverty and homelessness.
- Demonstrated knowledge of issues related to substance use and mental illness.
- Demonstrated ability to establish and maintain supporting and trusting relationships.
- Demonstrated ability to work in a manner that respects the value of diversity and principles of equity and anti-discrimination.
- Demonstrated ability to work independently in community settings with minimal supervision.
- Demonstrated ability to work collaboratively as part of a team.

- Willingness to work a variety of shifts; including night shifts throughout the week and on weekends; and on-call shifts as required.
- Post-secondary education in a relevant field which may include, Personal Support, Nursing, Community Development, or Social Work.
- Ability to speak multiple languages; including Cree will be considered an asset.
May be required to work in other locations in the Cochrane District.
