Position Summary
The Post-Majority Support Worker provides culturally grounded, trauma-informed support to youth and young adults (ages 18–25) transitioning from care to independence.
This role focuses on promoting stability, self-sufficiency, identity, and connection to community, while addressing barriers related to housing, education, employment, life skills, and wellness.
The position operates within a prevention-focused, strengths-based framework, aligned with Apitipi Anicinapek Nation’s vision of keeping youth connected to their families, culture, and community.
Key Responsibilities
1. Individualized Case Management
- Develop and implement individualized support plans in collaboration with youth
- Conduct ongoing assessments of needs, strengths, and goals
- Provide consistent one-on-one support and mentorship
- Monitor progress and adapt plans as needed
2. Housing & Stability Support
- Assist youth in securing and maintaining safe, stable housing
- Support transitions from care to independent or semi-independent living
- Advocate with landlords, housing providers, and service agencies
- Address risk factors related to homelessness or housing instability
3. Life Skills Development
Support youth in developing practical skills for independence, including:
- Budgeting and financial management
- Meal planning, cooking, and nutrition
- Time management and daily routines
- Navigating transportation and appointments
4. Education, Employment & Training Support
- Support access to education, training, and employment opportunities
- Assist with applications (school, jobs, funding programs)
- Connect youth to community resources and employment services
- Encourage long-term goal setting and career planning
5. Cultural Connection & Identity
- Facilitate access to cultural teachings, ceremonies, and land-based activities
- Support reconnection to community, family, and identity
- Engage Elders and Knowledge Keepers where appropriate
- Promote pride in culture and belonging
6. Advocacy & Systems Navigation
- Assist youth in
accessing services such as:
- Income supports (Ontario Works, etc.)
- Health and mental health services
- Education and training programs
- Advocate on behalf of youth to reduce systemic barriers
- Support youth in understanding their rights and responsibilities
7. Crisis Intervention & Support
- Provide short-term, solution-focused support during times of crisis
- De-escalate situations and connect youth to appropriate services
- Maintain professional boundaries while ensuring safety and support
8. Documentation & Reporting
- Maintain accurate and timely case notes and service records
- Track outcomes, service delivery, and program data
- Contribute to reporting requirements for funding bodies (e.g., ISC)
- Ensure confidentiality and proper file management
9. Collaboration & Team Participation
- Work collaboratively with Band Rep and Prevention staff
- Participate in case reviews and team meetings
- Build partnerships with community agencies and service providers
- Support integrated service delivery across programs
Qualifications
Education & Experience
- Education is an asset, but Life Experience is valued
- Diploma or degree in Social Work, Child & Youth Care, Indigenous Studies, or related field
- Equivalent combination of education and experience may be considered

- Understanding of:
- Child welfare system and post-majority supports
- Transitional challenges for youth aging out of care
- Indigenous culture, values, and community dynamics
- Strong case management and organizational skills
- Excellent communication and interpersonal abilities
- Ability to build trust and maintain professional boundaries

Working Conditions
- Travel required (regional and community-based)
