The Opportunity:

Job Title:                 Indigenous Palliative Care Planner & Coordinator                                   Salary Range: $75,000-$90,000

The Indigenous Palliative Care Coordinator will design, implement, and oversee an enhanced palliative care program to integrate cultural, spiritual, and clinical care for Indigenous clients. In collaboration with Elders, knowledge keepers, healthcare professionals, and community members, this role will ensure holistic and compassionate care at the end of life.

 Key Responsibilities

  • Develop and implement a culturally appropriate palliative care program in partnership with Indigenous partners.
  • Provide direct support to Indigenous clients and families, ensuring care plans respect cultural traditions, spiritual needs, and personal preferences.
  • Act as a liaison between clients, healthcare providers, and Indigenous organizations to coordinate seamless care delivery.
  • Collaborate with Elders and knowledge keepers to integrate traditional practices into palliative care.
  • Offer cultural competency training to staff on Indigenous approaches to health, wellness, and end-of-life care.
  • Organize community outreach initiatives to raise awareness about palliative care services.
  • Maintain accurate documentation of client care and program activities, ensuring privacy and confidentiality.

Qualifications

  • Post-secondary education in health sciences, nursing, social work, or a related field.
  • Experience working with Indigenous communities and understanding of their health, social, and cultural needs.
  • Familiarity with trauma-informed care, cultural safety, and reconciliation principles.
  • Strong communication, relationship-building, and advocacy skills.
  • Knowledge of palliative care principles and practices is an asset.
  • Valid driver’s license and reliable transportation for community visits.
  • Fluency in an Indigenous language is an asset but not required.
